81 Year Old Woman getting help from OC Hope Center

Recently, HOPE Center Dispatch received a call for service regarding an 81-year-old woman seeking shelter. HOPE Center Outreach Coordinators went to the Fullerton Train Depot where they were able to meet with the woman.

The woman was in need of a place to sleep, and she had a housing voucher which was about to expire.

The coordinators were able to arrange for the woman to go to the Fullerton Navigation Center, which provides beds, crisis evaluation, transportation services and more. After the coordinators provided the woman a snack and some water, she was transported to the center.

Our team and the Navigation Center’s staff is working to locate longer term housing for the woman.

Partner Spotlight – Outreach Grid

Outreach Grid is a dedicated unified platform that was designed to deliver better and faster care to persons experiencing homelessness. HOPE Center’s partnership with Outreach Grid enables the region to take on homelessness with a structured and collaborative approach, aligning and coordinating real-time efforts across multiple cities, on one unified platform.

How Does it Help?

Client and Case Management – We can track our work all in one platform i.e. encounters, phone calls, meetings, assessments, project enrollments, case notes, and documentation.

Encampment Management – With geolocation capabilities, they have developed a real-time map of encampments. We can log the status of encampments, create encampment groups and record work done for each location. We can also note encampment hazards and observations and update encampment status while we visit the location.

Outreach Grid Worker
Real-Time Shelter Bed Reservation System – Our Outreach Coordinators can reserve a bed for our clients. Once a shelter bed has been reserved, we are notified of the check in time and provide transportation to the shelter.

Dispatch Management Mobile Response System – Outreach Grid has developed the first of its kind dispatch system launched first in Orange County to deliver to persons experiencing homelessness the “right resources with the right people at the right time.” The dispatch system not only captures community members’ calls for homeless related services but also receives diverted calls from police departments. Dispatchers then assign the call to the right mobile team for the right resource in a timely manner.

Staff Spotlight – Aimee Hurtado

Aimee Hurtado is an OC Native who brings a wealth of experience and a genuine commitment to the community in her role as an Outreach Coordinator at the HOPE Center.

She holds a BA in Communications and has over 7 years of dedicated service in homeless services across the county. Aimee joined our team in May 2023 and is committed to serving the most vulnerable in the North County.

When she’s not working tirelessly to make a difference, Aimee enjoys volunteering in her community and exploring national parks.
